If you've played golf for any length of time, you've either experienced back pain or know someone who has. Lower back pain is the number one injury among golfers at every level, from weekend players to tour professionals.
The golf swing asks your spine to do something inherently stressful: rotate at high speed while maintaining a forward-bent posture, then decelerate rapidly. Repeat 60-80 times over 4 hours. Add the stress of walking, bending to tee up balls, and picking balls out of the cup, and your lower back accumulates significant load over a single round.
The good news: golf back pain is overwhelmingly preventable. The majority of cases stem from modifiable factors -- poor mobility, weak core muscles, and inadequate warm-up -- not from irreversible structural damage.
Why Golfers Get Back Pain
The Compensation Problem
The golf swing requires rotation through the hips and thoracic spine (upper back). When these joints are too stiff to rotate adequately, the body compensates by forcing the lumbar spine (lower back) to rotate beyond its comfortable range. The lumbar spine is designed primarily for stability, not rotation. Forcing it to rotate repeatedly under load is a recipe for pain.
This is why golfers with tight hips and stiff upper backs are far more likely to develop back pain than flexible golfers -- even if their swings look identical on video.
Compressive Force
Research using biomechanical analysis has measured compressive forces on the lumbar spine during the golf swing at approximately 6-8 times body weight. For an 80 kg golfer, that's up to 640 kg of compressive load -- on every single swing. Over 70-80 swings per round, the cumulative load is enormous.
Common Contributing Factors
- Tight hip flexors from prolonged sitting (desk jobs are a major risk factor)
- Limited thoracic spine rotation forcing the lower back to compensate
- Weak core muscles that can't stabilize the spine during rotation
- Inadequate warm-up before playing or practicing
- Excessive practice volume without adequate recovery
- Poor swing mechanics that increase spinal stress (reverse spine angle, early extension)
The Prevention Program
Daily Mobility Work (5 minutes)
These exercises maintain the hip and thoracic mobility that protects your lower back:
Cat-cow stretch: On hands and knees, alternate between arching your back (cow) and rounding it (cat). 15 slow repetitions. This mobilizes the entire spine and is the single best daily exercise for golfer back health.
Hip 90/90 transitions: Sit on the floor with both legs at 90-degree angles. Rotate your legs from one side to the other. 10 transitions. This targets hip internal and external rotation -- the exact movement pattern that protects your lower back during the swing.
Open book stretch: Lie on your side with knees bent. Rotate your top arm and torso open, following with your eyes. Hold 5 seconds, return. 8 reps each side. This targets thoracic rotation specifically.
Child's pose with rotation: From child's pose, place one hand behind your head and rotate your elbow toward the ceiling. 8 reps each side. Combines spinal decompression with rotation.
Core Strengthening (5 minutes, 3x per week)
A strong core stabilizes the spine during the rotational demands of the golf swing:
Plank: 3 sets of 30-45 seconds. The foundational core exercise.
Side plank: 2 sets of 20-30 seconds each side. Targets the obliques, which control rotational stability.
Dead bug: 3 sets of 8 each side. Lie on your back with arms and legs raised, then extend opposite arm and leg while keeping your lower back pressed to the floor. This teaches core bracing during movement -- exactly what the golf swing requires.
Bird dog: 3 sets of 8 each side. On hands and knees, extend opposite arm and leg. Develops the back extensors and core simultaneously.
Pallof press: 3 sets of 10 each side with a resistance band. Stand sideways to the anchor, press the band forward, and resist the rotation. This is the most golf-specific core exercise available.
Warm up before every round and practice session
Never swing a golf club without warming up first. Start with dynamic stretches (trunk rotations, hip circles, arm swings), then progress through gradually faster practice swings. Cold muscles and stiff joints absorb rotational forces poorly, increasing injury risk.
Address your hip mobility
Tight hips are the root cause of most golf back pain. If you sit at a desk for work, your hip flexors are likely shortened and your hip rotation restricted. Daily hip stretches (90/90s, deep squats, hip flexor stretches) directly reduce lower back compensation.
Build rotational core stability
Your core's job during the golf swing is to stabilize your spine while your hips and shoulders rotate around it. Train this with anti-rotation exercises (Pallof press, plank variations) rather than sit-ups or crunches.
Monitor your practice volume
More swings mean more spinal loading. If you practice 200 balls on the range and then play 18 holes the next day, your lower back has absorbed an enormous amount of stress. Build in recovery days and limit range sessions to 40-60 balls when playing the next day.
When Pain Appears
If you develop back pain, don't ignore it. Early intervention prevents minor issues from becoming chronic problems.
Mild discomfort during or after golf: Increase your mobility work, reduce practice volume, and apply ice after rounds. This usually resolves within 1-2 weeks.
Persistent pain (lasting more than 2 weeks): See a physiotherapist or sports medicine doctor. They can identify the specific cause and provide targeted treatment.
Sharp, shooting pain or pain radiating into the legs: Stop playing immediately and consult a medical professional. This may indicate a disc issue that requires specific treatment.
Pain that worsens despite rest: This is not a "play through it" situation. Get professional evaluation.
Swing Modifications for Back Protection
If you're prone to back pain, certain swing adjustments reduce spinal stress:
Slightly wider stance: Distributes load and reduces lateral spinal bending.
Slightly shorter backswing: A full backswing isn't necessary for good golf. Reducing your turn by 10-15% can significantly reduce lower back stress with minimal distance loss.
Slower transition: The most dangerous moment for the lower back is the transition from backswing to downswing. A smoother, less violent transition reduces peak compressive forces.
Stand taller at address: Excessive forward bend at setup increases lower back strain throughout the swing.
Equipment Considerations
Push cart vs. carry bag: Carrying a bag adds 10-15 kg of asymmetric load to one shoulder, increasing spinal stress. A push cart eliminates this entirely.
Ride when needed: There's no shame in using a cart on days when your back is stiff. A cart ride preserves your back for the swings that matter.
Ball retriever: Bending over 70-80 times per round (picking up tees, reading putts, retrieving balls) adds cumulative flexion stress. A ball retriever on your putter reduces unnecessary bending.
